Keep what the team learned
An evaluation ends with a submitted report and an invoice, and the team moves on. What the team learned about its own work leaves with it: the instrument that took three rounds, the partner who delivered, the review process that cost a month. Six months later a new team makes the same mistakes.
This template gives you a 90-minute review in six steps. Four plain questions drive it: what did we expect, what happened, why the difference and what will we change. Prompts for each phase of an evaluation take the group past the first answer, and a facilitator script handles the drift towards blame. You leave with at most five changes, each with an owner, a date and a document where it will live.
It closes the loop on the Statement of Work and the Inception Report, where most of the lessons are recorded as changes. Pair it with the Client Feedback Survey & Debrief, which gives you the client's side of the same story.
